You do not need to fill out the form, so nothing as a company name. Just give them a call and tell them you downloaded the direct deposit form, but your payroll department is requesting the bank address. They'll give it to you, no problem. Then you can use that address, with the account number and routing number, to initiate your ACH withdrawal. I hope this helps, and you are not being a pest! Glad I could help!!
