If you refer a friend to Netspend and they load $40, Netspend will load $20 on each of your cards. You have to use the link they send in your registered email. That's an offer you cant refuse, also there is no limit to how many friends you get to sign up

update for June 2016
greendot.com has no charges for transactions and if you load $1000. a month they waive monthly charge of 7.95, they do charge international fees of about 3% per transaction (about half of the casinos) and to load cash is $3.95 to $4.95 each but thousands of places to load at.
Rush card has no transaction fee plan for 5.95 a month and I've only had international fee on one casino out of about 15 I've deposited at. Same load fees and locations as Greendot.
Both have free load with direct deposit.
